Primary Responsibilities
As a Palliative Care Assistant, your daily activities and key performance areas will include:
- Holistic Patient Care: Deliver high-quality, clinical care detailed in patient care plans, assist with clinical procedures, record vital observations, and manage nutritional and hydration needs.
- Day-to-Day Support: Assist patients with washing, dressing, self-care, and pressure area care using repositioning techniques and SSKIN care bundles.
- Reporting & Documentation: Promptly record and communicate changes in patient conditions to the Registered Nurse, tracking fluid outputs, bowel movements, and updating patient records accurately.
- Advanced Care Assistance: Manage nebulized medication administration, monitor syringe drivers, check skin conditions around needles, manage transdermal patches, and securely handle oxygen equipment following proper training.
- Service User Interaction: Support family members and significant others with deep empathy and dignity, while actively advocating for the patients under your care.
- Operational Adherence: Maintain the absolute highest standards of infection control, health and safety protocols, and strict patient confidentiality at all times.
